TX Recruiting is partnering with a growing hospice organization in The Woodlands, TX to hire a compassionate, experienced Hospice RN. You’ll deliver skilled nursing care, build individualized care plans, and collaborate with an interdisciplinary team to keep patients and families comfortable and cared for.

Responsibilities

• Manage a caseload of hospice patients and provide skilled nursing care in homes, facilities, and other care settings.
• Complete thorough assessments and develop individualized plans of care.
• Deliver symptom and pain management in line with each patient’s plan of care.
• Educate patients, families, and caregivers on the plan of care, the disease process, and what to expect in end-of-life care.
• Coordinate closely with physicians and the interdisciplinary team — social workers, chaplains, aides, and volunteers.
• Document visits and care accurately and on time.
• Follow hospice policies, clinical standards, and regulatory requirements.
• Participate in the on-call rotation as needed.

Qualifications

• Current Texas RN license.
• Hospice or home health experience strongly preferred.
• Solid knowledge of hospice care, symptom management, and end-of-life support.
• Strong clinical assessment, communication, and documentation skills.
• Compassionate, self-motivated, and comfortable managing a caseload independently.
• Valid driver’s license and reliable transportation.
• Organized, dependable, and comfortable in a fast-paced environment.
