TX Recruiting is partnering with a growing hospice provider to hire an experienced Hospice Liaison in The Woodlands, TX. We’re looking for a relationship-builder who knows the North Houston healthcare market and can turn strong connections into hospice referrals and admissions.

What You’ll Do

• Develop and grow relationships with physicians, hospitals, SNFs, assisted living communities, case managers, social workers, and other referral sources.
• Generate new hospice referrals and help grow patient census across The Woodlands and the surrounding territory.
• Educate referral partners on hospice services, eligibility, and the benefits of hospice care.
• Identify and open new accounts and opportunities within the market.
• Consistently work toward monthly referral and admission goals.
• Partner with the clinical team to ensure a smooth transition from referral to admission.

What We’re Looking For

• Hospice or home health sales/liaison experience strongly preferred.
• Established relationships within The Woodlands / North Houston healthcare market are a major plus.
• A proven track record of developing referral sources and generating admissions.
• Strong relationship-building and communication skills.
• Self-motivated and comfortable managing a territory independently.
• Valid driver’s license and reliable transportation.
